PATIENT INFORMATION

First Visit To Le & Savko Dental Santa Rosa, CA

In most cases your initial appointment with us will consist of a comprehensive dental examination that includes an oral cancer exam and a full series of digital radiographs necessary to evaluate your dental health.  Emergency situations requiring immediate treatment will be done the same day if time permits.  However in cases that involve a complex medical history or extensive treatment may require an evaluation and a second appointment to provide treatment.

Please assist us by providing the following information at the time of your first dental visit:

  • Completion of necessary Health/Medical History information including a list of medications you are currently taking.  At the time of scheduling your appointment please alert the office if you have a medical condition that may affect your dental care (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or If you require antibiotic pre-medication for dental treatment (to prevent bacterial endocarditis).
  • A breif description of your previous dental treatment and a summary of how you would like your smile to be.
  • Financial Information necessary to properly establish your account with our office
  • If you have dental insurance, please bring the card provided you or have the insured persons information necessry for billing. Please note it is the responsibility of the patient to verify that their coverage will work when receiving treatment in our office.  In most cases this can be done by speaking with either the Human Resources Manager in your office or by contacting the insurance company directly.
  • Most recent dental xrays if available.

X-Rays

If your previous dentist has taken recent x-rays (within 12-months), we ask that you request they forward them to our office by sending them to Le & Savko Dental Care, 55 Mission Circle, Suite 105, Santa Rosa, CA 95409. If they are digital they can forward to them to us via e-mail at lesavko@sonic.net. If they are not digital and there is not enough time, please pick them up and bring them to our office. If additional xrays are necessary, they will be taken at our office at your initial visit.

IMPORTANT NOTE: A parent or guardian must accompany all patients under 18 at the initial visit.
